Interesting question. I recently read a fanfic (the title of which escapes at the moment) wherein Scully had a concussion from when Pfaster ran her off the road, and was thus hallucinating when Pfaster shifts into the demon, etc.
That made me think. The suggestion is plausible. When I first saw the episode, I didn't delve too deeply into the why, but had thought that Pfaster represented all Scully's fears rolled into one ball of nastiness, highlighted by the car accident, her situation, etc.

Your thoughts?

Gosh, I can't describe how much I love this episode. And this artwork is so good that it makes me want to watch it again. I love the moment when Scully breaks down and Mulder pulls her into his embrace. Love it.
